To be eligible for the Bright Futures Scholarship, students must complete the hours requirement either through volunteer service hours OR paid work hours. The student can also use a combination of the two to meet the requirement (if the student chooses to use a combination, then they must total 100 hours combined for any of the scholarship designations).

Complete & Return Community Service Hours Form to Your Counselor

Volunteer/Service Hours:

• Fill out all pages of the Bright Futures Community Service/Paid Work Proposal Form (document available below)
• Make sure to have your parent/guardian sign and date page 3 "Parent Signature" line
• Make sure to have a contact from the organization where the service was completed to sign and date page 3 on the "Business/Agency Contact's Signature" line  
• Submit the completed proposal form to your school counselor

Paid Work Hours

• Fill out all pages of the Bright Futures Community Service/Paid Work Proposal Form (document available below)
• Make sure to have your parent/guardian sign and date page 3
• Make sure to have your manager/supervisor from your place of work sign and date page 3 on the "Business/Agency Contact's Signature" line
• Log work hours and dates on page 3 of the form
• Submit a copy of the pay stub(s) for each date/hour listed on page 3 with the completed form to your school counselor.
