Eligibility Checklist: HOW TO BECOME A ST. CLOUD HIGH SCHOOL ATHLETE:

1. Register on Athletic Clearance
2. Students must complete a Sports physical.  Health Department physicals will no longer be accepted.  Physicals must be completed on FHSAA EL2 Form.
3. Students must upload page 4 only to Athletic Clearance.  EL3 electronic signatures must be completed.
4. Students must complete an ECG. (One ECG will be used for all of high school).  Incoming freshman ECG must be completed AFTER 4/1/2025.  If you previously participated in a sport at St. Cloud High School, and already have an ECG, we will upload it for you.
5. Students must upload ECG Form to Athletic Clearance.
6. Students must complete Impact Concussion Baseline Testing on a laptop or computer AFTER 6/1/2025. (Website: www.impacttestonline.com/testing) (Launch Code: FF0Y2TLL9N) *Code includes a zero, not the letter "O".
7. NEW MANDATORY: To be cleared by FHSAA, students must complete NFHS Learn Courses. Students must upload certificates (dated after 6/1/2025). 
   1. Concussion for Students
   2. Heat Illness Prevention
   3. Sudden Cardiac Arrest
   4. Sportsmanship
8. Students must upload a confirmation page that is signed by parents and students.  IMPORTANT: Select ALL SPORTS that you are interested in.  Otherwise, you will have to upload a separate confirmation page for every sport added at a later date.
9. Upload the Following
   1. Physical Page 4 (2025-2026 Athletic Packet)
   2. ECG (if applicable) (ECG Form)
   3. Pay $35 fee (Athletic Clearance)
   4. NFHS Course Certificate (four courses) (NFHS Learn Courses)
   5. Confirmation Page (with Signature)

All athletics packets are available from your Coach, can be printed by clicking the links found above, or you can email me @ bryan.smart@osceolaschools.net and request one. Remember all summer conditioning programs are voluntary. While we want you to be a part of what we're doing in preparation for the new school year, we certainly understand the right of your parents to do what they feel is in your best interest.
